Infertility and ART

As doctors, we do understand the sentiment of a couple who has been trying long to start a family. We encourage couples to undergo ART treatments when they have not been able to conceive for more than a year taking in to consideration age, sperm count, egg quality etc.

We recommend you to choose ART when the couple is still young and we have the possibility of getting good eggs and sperms.

For men, In case of zero sperms in ejaculate, hehas to undergo TESA or Micro-TESA to retrieve sperms which is used to fertilize eggs. This is a surgical procedure done under anaesthesia. The man is fit to travel after 72 hours.

Intrauterine insemination (IUI)

An simple procedure that puts sperm directly inside your uterus, which helps healthy sperm get closer to your egg.

IUI works by putting sperm cells directly into your uterus around the time you’re ovulating, helping the sperm get closer to your egg. This cuts down on the time and distance sperm has to travel, making it easier to fertilize your egg.

Before having the insemination procedure, you may take medicines that stimulate ovulation. Semen is collected from your partner or a donor. It goes through a process called “sperm washing” that collects a concentrated amount of healthy sperm from the semen.

Then your doctor puts the sperm right into your uterus. Pregnancy happens if sperm fertilizes your egg, and the fertilized egg implants in the lining of your uterus.

Process

During the IUI procedure, the doctor slides a thin, flexible tube through your cervix into your uterus. They use a small syringe to insert the sperm through the tube directly into your uterus.

The insemination procedure is takes about 5-10 minutes. You don’t need anesthesia. You may experience mild cramping.

IVF- In Vitro Fertilization & ICSI- Intracytoplasmic sperm injection.

Two of the most common fertility treatments are In Vitro Fertilisation (IVF) and Intracytoplasmic Sperm Injection (ICSI).

What is IVF?

IVF places your eggs with prepared sperm in culture media within a laboratory dish. The sperm will naturally penetrate the egg leading to fertilisation overnight. The resulting embryos will develop in an incubator for three to five days. Your embryos are monitored by an embryologist and the best will be chosen for transfer to your womb.

What is ICSI?

With Intra-Cytoplasmic Sperm Injection (ICSI) a single sperm is selected by an embryologist, taken up in a fine glass needle, and injected directly into each egg. Your fertilised egg (embryo) is then transferred into your womb in the same way as an IVF cycle.

The key difference between IVF and ICSI is how the sperm fertilises the egg. In IVF, the egg and sperm (of which there are multiple) are left in a petri dish to fertilise on their own. In ICSI, one sperm is directly injected into the egg.

Process

Daily Injections called Gonadotropins from Day 2 of menses cycle, to stimulate the production of more eggs.
Egg retrieval or egg pick-up, done under sedation in Operation Theatre. Sperms are collected either from ejaculate or surgically and washed.
Inseminate each egg in a petri dish or do ICSI.
Culture the resultant mix for 2 to 6 days.
Embryo transfer; done awake on Day-2 to day-6 post egg pick-up.

Planning

Should start in your home country. All medical records till date should be made available for a quick and positive outcome.
Basic tests and work up may be done prior to visiting us so as to reduce your days of stay in India .
The required test details and other examinations prior to an IVF treatment will be communicated via e-mail, or Whatsapp (group involving your facilitator and patient along with doctor or a closed group with the doctor and the patient alone)
Based on the results of the tests done, we can take corrective measures if required prior to reaching India.

Treatments cost are different based on the protocols chosen for egg stimulation as per patient profile. You can discuss with our doctor as to which protocol might give you the best results. The cost for the procedure and details of what is included will be discussed by our patient coordinator.

Treatment duration

From the Day-1 of menstrual cycle till 3 weeks. Provided all initial work-up has been completed prior.
ICMR allows us to freeze excess embryos if produced, after a fresh IVF-ET (embryo transfer). The duration of freezing is for six months and to be renewed every six months till the fertility seeking couple decide to discard the embryos.
In a single attempt, not more than three embryos would be transferred and preferably, only two embryos. This is to prevent higher order multiple pregnancy and its inherent risk of abortion.
“Carry home baby rate” across the globe is not more than 55% and our centre too has the same track record.

Misconceptions

We do not use third party gamete unless requested by the couple.
There is never a mix-up of gametes. Utmost care is taken in sample tagging.
Almost always couple wish to avoid IVF-ET, except perhaps in Zero sperms in ejaculate on repeated testing and or if the lady had her tubes removed due to Tubal pregnancy.

Common doubts

Why is the procedure relatively expensive?

The drugs involved in controlled ovarian stimulation are expensive.
The consumables like the micro dishes, micro pipettes, ICSI holding pipette and injecting needle, the Nunc wells etc are very delicate. They are each tested for “embryo safety” and all are imported. They are very thin (eg: thin as 70 micron= 0.07 mm in diameter).
The culture media is imported from Europe or Australia and are to be stored in particular conditions even in transport. They cannot be re-used.

Why doesn’t every one become pregnant?

Age may be advanced.
They could have genetic factors.
Uterus could have been damaged by diseases or surgical scar.
Males may not have sperms at all.
